Error uploading a CSV file with the View Option

I get the following error after choosing "Finish".
DBAction.doUpdate() exception
I'm uploading to a mysql database and originally got an error message instructing me to try changing the precision but that didn't help. When my original file didn't work, I decided to try a simple file (see attached) and got the same error message.
System info below:
