Thanks for reaching out. Without seeing exactly where these were moved to and what each looks like it's hard to specify the likely exact method by which one or both of these were altered, but generally, customizing the Logout button can be done in a couple different ways. I'd check the possibilities found in both the Custom Header & Footer and Custom Images & CSS Wiki entries. If you read through each of these and follow the steps on how to perform each you should be able to look in those respective locations for references to these changes. Alternately, if you're aware of the party responsible for creating the customization you should be able to find this out from them as well. Please let me know how goes and whether you have any additional questions.
