How to create new custom login page as per requirement ?

Hi Yellow-fin team ,
I want to create new login page with login Id and password,i want to place login id & password in different places [ Ex : i want to change middle ]
I want to change logo and background image also
If again i can create new login page (or) if any modification can i do in application ?
If any video is there,for changing login page
